How We Deliver Care at Relax Healthcare
Initial Referral
When you first recognised a need for care, you may have contacted Relax Healthcare directly, or you may have been referred to us by your local social services department, which has agreed to contribute towards the cost of your care. Whichever route you’ve taken, we treat all personal information with the utmost sensitivity and confidentiality. Before we begin providing any care services, we’ll speak with you—the person receiving the care—alongside your carer, if applicable, and any professionals involved, such as social services. It’s important to us that we only offer services which are truly suitable and beneficial for your individual needs.

Needs Assessment
If your referral has come via a local authority, their care manager will already have completed a needs assessment outlining the support required. A summary of this assessment will be shared with us. However, if you have contacted us independently, we will carry out our own assessment. This process involves asking you a series of questions, and we may need to gather relevant information from your carer, GP, or other healthcare professionals who know your situation well. Our trained staff will handle this with sensitivity and discretion. We aim to understand your needs as thoroughly—and as tactfully—as possible. While the process may feel detailed, it is essential for us to offer care that fits you personally. All information you share will be handled in complete confidence.

Risk Assessment
Receiving care at home offers independence and familiarity, but it can also involve some level of risk, especially as care workers are not present 24/7. Many people find that this balance is worthwhile, but it is essential that everyone involved understands and accepts any risks. We will work with you to complete a thorough risk assessment, ensuring that any potential concerns are recognised and addressed. Where possible, we will suggest practical ways to reduce or manage unnecessary risks without compromising your independence.

Your Care Plan (Service User Plan)
Once we’ve completed the needs and risk assessments, we will, together with you and your carer, create a personalised care plan—known as the Service User Plan. This plan outlines exactly what services we will provide, including details such as the timing of care worker visits, the tasks to be undertaken, and any goals or outcomes we aim to achieve together. You are central to this process, and your preferences will shape the care we deliver.

Ongoing Reviews
Your circumstances and requirements may change over time. Whether your care needs increase, decrease, or simply shift in focus, we will regularly review your plan with you to ensure it continues to meet your needs. If at any point you feel something needs to change—whether big or small—please don’t hesitate to tell us. We are here to adapt and support you.

Fees
If you’ve been referred through a local authority, your care fees will typically be funded by them. We also work with clients who receive care through a direct payment scheme. In such cases, we charge no more than the standard rate agreed with social services.
